Note: You will be asked to select a Primary Care Physician (PCP) when you enroll in any HMO plan. If you do not select a PCP, one will be assigned to you. With an HMO plan, you must use providers in the network. With an HMO-POS plan you may see an out of network provider if that provider accepts Medicare assignment and you as a patient. You may be responsible for any or all additional fees or services provided by doctors or hospitals out of the network. In some networks, PCPs and Specialty Physicians are grouped by both geographical location and practice referral patterns and referred to as a Local Physician Organizations (LPO). If your network has an LPO that your selected PCP is associated with, then that will be your LPO. If you require services from a contracted provider who is not in your LPO, please contact the plan. Please refer to the plan's Evidence of Coverage document for more detailed information. Providers in the network can change at any time. For Today’s Options PPO: Out-of-network/non-contracted providers are under no obligation to treat Today's Options® PPO members, except in emergency situations. For a decision about whether we will cover an out-of-network service, we encourage you or your provider to ask us for a pre-service organization determination before you receive the service.
